Abstract
Maintenance of chronic illnesses, such as chronic obstructive pulmonary disease (COPD), in the primary care setting can be arduous due to the complexity of the illness and the various guidelines from multiple organizations. The purpose of this project was to determine if bundling care for patients with COPD using the Global Initiative for Chronic Pulmonary Disease (GOLD) guidelines can improve adherence to the management and maintenance of COPD by nurse practitioners (NPs). This project consisted of review of 67 charts with COPD documented by nurse practitioners to determine if they included key elements for COPD maintenance. Also a test was administered to assess their knowledge and clinical practice of COPD. Following a training program of the GOLD standards and implementation of a bundled care tool, the nurse practitioners were evaluated again. Following the training program, the charts were reviewed to determine if the training program improved their adherence to the GOLD standards. More key components of COPD were documented by the nurse practitioners after education and tool implementation (p < .001). Additionally, scores on the post-test were higher than scores on the pre-test, demonstrating that the education improved knowledge and practice of COPD (p = .023). This project successfully determined that a bundled care tool in the primary care setting can be applied for COPD maintenance. The test scores imply that NPs are knowledgeable about COPD and correctly managing the condition; however, implementing knowledge into practice was difficult but improved after the education was provided.
